Token Prizes - Tokens can be exchanged for prizes or for Full Mouth Bleaching (below)!!!

WAYS TO EARN TOKENS

  • Be on time for regularly scheduled appointments with clean teeth and NO broken brackets, bands, or wires (1 Token)
  • Wear your appliance as instructed (Rubber Bands, Forsus Springs, Head Gear, etc.) (1 Token)
  • Wear your Dr. Leiker T-shirt to your appointment (1 Token)
  • Visit your General Dentist for your 4-6 month cleaning and bring us the receipt or a note from their office (5 Tokens)
  • Be seen by Dr. Leiker or staff outside the office wearing your Dr. Leiker T-shirt - football game, the mall, movie theater, etc. (5 Tokens)
  • Refer a patient to the office. Be sure they let us know at their initial exam that you referred them!!! (5 Tokens)
  • The New Patient you referred starts active treatment:
    • One arch or Phase 1 Treatment (10 Tokens)
    • Full Treatment (20 Tokens)
    • iBraces Treatment (35 Tokens)

* Tokens are not given out for unscheduled appointments, emergency appointments, or repair appointments.
* Only patients in active treatment, Phase 1, or Full Treatment, are eligible for receiving tokens. Growth monitoring and retention patients can still receive tokens for wearing their T-shirts for appointments or outside office.
